Steps To Become A New Patient

You will be asked to submit a photo of your insurance card & government issued ID.

Submit your information by filling out our online intake. Your application will be carefully reviewed by our team.

2. Complete Forms

Easily complete forms in your online patient portal, without having to come into our office. These forms consist of your medical history, current symptoms, and office policies.

3. Get Scheduled

We will find a day and time that works best for you.

Integrative Psychiatry

At Paradise, we believe in integrative medicine for mental health. We take a holistic approach that looks at all aspects of the patient’s mental, physical, and spiritual life. Every patient gets a full lab work-up that includes genetic testing. This approach helps us find out exactly how we can best tailor a treatment plan that is unique to your DNA.
