Steps To Become A New Patient

You will be asked to submit a photo of your insurance card & government issued ID.

1. Submit Application

Submit your information by filling out our online intake. Your application will be carefully reviewed by our team.

2. Complete Forms

Easily complete forms in your online patient portal, without having to come into our office. These forms consist of your medical history, current symptoms, and office policies.

3. Get Scheduled

We will find a day and time that works best for you.

Preparing For Your First Appointment

We are happy you chose Paradise Behavioral Health to help you on your mental health journey. Please remember to bring the following items with you to your first appointment. If you forget to bring in any of these items, your appointment may need to be rescheduled.

1) Your insurance card

2) Payment for your copay or self pay rate

3) Your medication list

4) Your medical records (if available)

How do I become a patient?

1. Complete an application.

2. After one of our team members has reviewed your intake form, you will be sent a link to set up a patient portal account. All of the new patient forms will be available for your review and signature in your patient portal.

3. Schedule an appointment.

Can I become a patient if I live out of state?

Paradise cannot provide any service for out of state residents. For a patient to be considered a Florida resident they must have a valid ID with a Florida address and their pharmacy must be in Florida. 

For patients that have multiple residences in different states, they must prove they have a Florida address by providing a government ID or utility bill AND have a pharmacy in their chart within the state of Florida.

What will be expected of me as a patient of Paradise Behavioral Health?

You are serious about taking getting well and living an optimal lifestyle. You will make every effort to take our recommendations knowing we have your best interest in mind. Paradise Behavioral Health offers a holistic approach to wellness, not just medications.

Is telemedicine covered by insurance?

Why do I need a patient portal account?In order for us to enhance communication and patient experience, you will be required to register for a Patient Portal. If you are a new patient, we will send you an e-mail with a link to start the registration process.  

Use your patient portal to:

– Request refills

– Message your provider

– View your chart records

– Pay your bill

– Sign forms
